Service Description

Digitally Stored and/or Transmitted by the patient and/or caregiver to the physician or other Qualified healthcare professional, qualified by education, training, licensure/regulation (when applicable). A physician or other qualified healthcare professional collects and interprets physiologic data. The data (e.g., blood pressure) is Stored Digitally and transmitted by the patient and/or the caregiver to the provider. Physician will review it and generate a report with the result and made care plan changes needed based on RPM.
